Men’s Health Co.

Telehealth involves the delivery of healthcare services using electronic communications, information technology, and digital platforms between a healthcare provider and a patient who are not in the same physical location. By utilizing the clinical services of Men’s Health Co., you acknowledge and consent to the following terms:

1. Nature of Telehealth

You understand that your medical evaluations, laboratory orders, and clinical follow-ups will be conducted electronically. This may include secure messaging, asynchronous clinical questionnaires, audio calls, or interactive video consultations.

2. Expected Benefits

  • Improved and convenient access to medical care for men’s health conditions.
  • Efficient evaluation and management of your treatment protocols without requiring physical travel to a brick-and-mortar office.

3. Anticipated Risks & Limitations

While telehealth is highly effective, you acknowledge that there are inherent risks:

  • Technical Limitations: Interruptions, unauthorized access, or technical difficulties could disrupt your consultation or access to data.
  • Clinical Limitations: Because a provider cannot perform a physical, hands-on examination, there are limitations to what can be evaluated. If your provider determines that telehealth is insufficient for your clinical needs, they reserve the right to direct you to an in-person physician.
  • Information Accuracy: The quality and safety of your treatment depend entirely on the honesty and accuracy of the health history and laboratory data you provide.

4. Privacy & Security

Men’s Health Co. utilizes secure, encryption-protected, and HIPAA-compliant digital infrastructure to protect your personal and medical information. However, you are responsible for securing your own device, password, and local network to prevent uninvited third parties from viewing your medical portal.

5. Patient Acknowledgement & Consent
